Error: Page Cannot be Displayed (Entrust IdentityGuard ISAPI)

Root Cause: After entering second factor credentials using the Entrust IdentityGuard ISAPI filter, a “Page cannot be displayed” error is presented                          to the user.
Findings: 
This can happen if the Entrust IdentityGuard ISAPI filter has been installed, uninstalled, and reinstalled a second time. The last event in the filter log will be “Calling isAuthenticated”, and an application error will be seen in the event viewer similar to the following:  
Faulting application w3wp.exe, version 6.0.3790.1830, faulting module IdentityGuardAuthValidationServiceClient.dll, version 8.1.10.134, fault address 0x000011e3.
